Notice of Change of Address (Form MVR-24A)
Perhaps one of the easiest DMV forms, the Change of Address Form (Form MVR-24A) is used by all DMVs across the country to keep up-to-date records of you and your car’s location. It is your responsibility to notify the DMV whenever important information, such as your legal name or address, change. Your drivers license is your primary form of identification, and thus must have accurate information at all times. Failure to notify the DMV of an address change can result in monetary fines after a certain period of time.
North Carolina Title Application (Form MVR-1)
One of the most important and commonly used DMV forms is the Title Application Form (Form MVR-1), which is required whenever you purchase a vehicle, new or used. A car’s title acts as your legal proof of ownership, and is required should you ever want to sell or transfer your vehicle. It is also required that your automobile be re-registered annually, but these renewals can be handled online or via mail. Registering a vehicle for the first time needs to be done in person at the DMV. This is such an important process that you need to turn it in shortly after completing your car purchase, or there may be late fees incurred to you for not registering it on time.
Application for Handicap Driver Registration Plate (Form MVR-37)
If you have a legitimate disability which impairs mobility, you qualify to park in spaces specifically reserved for the handicapped, which is especially convenient in crowded parking areas. To acquire this placard, you and your primary doctor will need to fill out the Application for Handicapped Drivers Registration Plate (Form MVR-37) and turn it into the DMV. Your physician will need to describe the nature and duration of the condition, and the DMV will provide you with the special license plate for the amount of time deemed appropriate by the doctor. Abuse of the handicap plate system is taken very seriously, and your plate will only be considered valid for as long as your physician states you need it.
